Ephedrae Herba is one of the prescriptions of Yuehpi-chiachu-tang. Major compositions of Ephedrae Herba, ephedrine alkaloids, are banned substances under the World Anti-Doping Agency. The purpose of this study was to investigate the amounts of ephedrine alkaloids in Yuehpi-chiachu-tang and the elimination of urinary ephedrine alkaloids after administering of Yuehpi-chiachu-tang. The study was divided into three groups, the first group had eight healthy volunteers and took a single dose of Yuehpi-chiachu-tang. All urine was collected for 48 hours after administration. The second group had eight healthy athletes and took a therapeutic dose of the Yuehpi-chiachu-tang three times per day for three days. All urine was collected for six days after administration. The third group had five healthy athletes and took a therapeutic dose of after Yuehpi-chiachu-tang three times per day for six days. All urine was collected for nine days after administration. Urinary ephedrine alkaloids were analyzed by liquid chromatography tandem mass spectrometry. The results showed that ephedrine, pseudoephedrine, norpseudoephedrine, norephedrine, and methylephedrine do excrete in urine after taking Yuehpi-chiachu-tang. The estimated elimination half-lives of ephedrine alkaloids were 6 to 9 hr, and the time to reach maximum concentration of ephedrine alkaloids following single-dose administration were 7 to10 hr. The highest concentration of ephedrine alkaloids was ephedrine (337.5±94.95 ng/mL), which was lower than the threshold of World Anti-Doping Agency (WADA)-10μg/mL. The norpseudoephedrine had highest concentration in urine after three or six days of multiple-dose of Yuehpi-chiachu-tang administering, with 2550 and 2200 ng/mL, respectively. There were similar concentrations of ephedrine alkaloids between three and six days of multiple-dose administering and was lower than the threshold of WADA. This study concluded if the athlete takes the Yuehpi-chiachu-tang preparation according to the drugs instruction ( 1.5g, three times a day) for short-term (six days) in competition would not lead to positive doping result.
