Description
Summary:碩士 === 國立陽明大學 === 醫務管理研究所 === 98 === Background: Prenatal care is a common medical service in Taiwan. It can improve the health status of both mothers and infants, prevent the risk factors and have interventions in the early stage of pregnancy. Generally, doctors prefer to perform virginal delivery for women unless they have some complications when pregnancy. The objective of this study is to investigate the correlation of prenatal care utilization and delivery types. Method: This study analysed an one-million population data (year 2006-2007) of claim database from National Health Insurance. Women who were 15-50 year-old and had delivery in 2007 were included in this study. The study collected the prenatal care records or these women retrospectively and deleted the missing data. There were 7750 cases in this study eventually. This study classified prenatal care utilization into 3 groups to explored the correlation of prenatal care utilization and delivery types by Logistic regression. Results: Prenatal care is very acceptable in Taiwan. There are 65.7% women in Taiwan who had adequate prenatal care visits. About 2613 women (33.72%) underwent cesarean section when delivery which conform to the prevalence rate of cesarean section in Taiwan. This study found that women who use prenatal care for 7-10 visits are tend to undergo cesarean section. Besides, women with higher age, higher income and was delivery in obstetrics clinics are tend to undergo cesarean section. Discussion: Prenatal care is a well-known medical service in Taiwan. This study found that women who had 7-10 visits of prenatal care prefer to undergo cesarean section. Age, personal income and level of delivery place are also factors to affect delivery types. Here are some suggestions of this study: (1) improve the health education function of prenatal care. (2) encourage obstetrics hospitals and clinics to perform vaginal delivery. Hope these could reduce the cesarean rate in Taiwan.
