Description
Summary:碩士 === 國立陽明大學 === 生醫光電工程研究所 === 98 === Varicocele is an abnormal enlargement and dilatation of the veins of the pampiniform plexus within the spermatic cord and is one of the causes of male infertility. It also is a common disease which will affect about 10-20% of all males and occurs frequently around the time of puberty. However, this disease develops slowly and may not have any symptoms. So, the confirmation of varicocele is often diagnosed lately. Recent studies have shown that oxidative stress might play an important role in patient with varicocele, and more oxidative damage might occur in spermatic veins. Does it bring hypoxemia in the target blood vessels? According to the advantages of biophotonics, They are noninvasive, relative inexpensive, detect technologically simple and fast, moreover, pose no risk factor. And Diffuse Reflectance Spectroscopy (DRS) has been successful applied to detect total hemoglobin concentration and oxygen saturation of biological tissues in several studies, such as brain, oral oncology, breast tissues and colon-rectum cancer, etc. So, how about varicocele? In this study, we use our DRS system devising a series of experiments to prove the device could be used in this topic and try to find the differences of the oxygen level in blood between normal and abnormal tissues with varicoceles. The results in clinical show that compared with normal subjects group, although the total hemoglobin concentration (THC) has a decreased situation in both sides with varicocele but it do not have remarkable variations in other two groups. And the oxygen saturations (StO2%) have a little decreased in varicocele with both sides and right side groups. And the group of left side in varicocele shows no any change. We believe we should immobilize a range, such as the patient with infertility, and change the experimental method with lie-detecting to stand-detecting, that we might obtainclearer conclusion. We also measured a varicocele in left testis with pre- and post-surgery and could find the StO2% has a little raise after the surgery. But it was the only case, so we think we should increase the measurements around the surgery procedures and compare the results. To summarize this research, although there are some analytic results conform to the expectation. But we still cannot have a strongly conclusion to explain all the data we got. Further studies need to collect more specific data to find and confirm the results and its relationship between normal subjects and varicoceles. We also believe that it is a novel technique and will be a powerful medical tool using diffuse optical method to detect the oxygen level in blood of this disease.
