In order to get good medical care, need good interaction between physicians and patients. But health care providers understand than the patients in the medical domain. This phenomenon is known as medical information asymmetry. Family physician system is not currently popular. People can not get integrity, sustainability and convenience of health care and health education. Health information what distributed in different hospital make health knowledge is decentralized. This research follows the knowledge management process to build system. User manages own health record. Learning from doing. Hope to improve health literacy. We build a knowledge platform for storing personal health record history on network. It store allergies, medication history and treatment information. In the knowledge management process what system using. User will collect and manage our health records. Knowledge structure is build by ontology engineering. Describing the knowledge is controlled by resource description framework (RDF), it stored in the knowledge base. Knowledge can exchange and utilization. User manage own personal health record to enhance patient health literacy. It is to be hoped medical care can be changed a doctor's advice and instruction to Service of collaboration and understanding.
