Approximately 20% of pleural effusions are caused by malignancy and in 10–50% of cancer patients may be the initial presentation. One of the common difficulties in effusion cytology is the distinction between cancer cells and reactive mesothelial cells. Moreover, metastatic adenocarcinoma cells often are associated with prominent mesothelial hyperplasia, making the diagnosis more difficult. Pleural effusion cytology findings are positive in only 60% of cases on average. Thus, many additional methods have been evaluated to improve the diagnostic accuracy and avoid invasive diagnostic techniques. CAIX (carbonic anhydrase IX) and GLUT-1 (glucose transport-1) are undetectable in most normal epithelial tissues and benign epithelial tumors but are expressed in a significant proportion of a variety of human carcinomas, including lung cancer, breast cancer, and colon cancer. Therefore, we collect 104 samples to evaluate CAIX and GLUT-1 protein expressions by immunocytochemical stain(ICC) and using enzyme-linked immunosorbent assay (ELISA) to evaluate CAIX protein value in pleural effusions. And then we got the results that the sensitivity /specificity of CAIX and GLUT-1 were 63.8%/100% and 74.5%/100% in detecting malignancy compared with conventional cytological examination. To set up cut-off value at 2000pg/dl then the sensitivity/ specificity was 63.6%/89.8%. These results will develop a feasible new test in order to improve the limitations of the morphological approach and the detection rate of malignancy in pleural effusion.
