Pilots are the very first frontier for flight safety in the aviation family. The objective of this study is to utilize their knowledge and flight experience to analyze and identify the potential risk factors in each flight and find out the relationship between those factors. Based on the results of this study, we then setup the strategies to avoid or reduce the possibility of making errors. In this study we apply the Analytic Network Process, ANP theory, and Decision Making Trial and Evaluation Laboratory, DEMATEL to analyze those potential risks. After constructing the relationship between those safety factors and their importance, we propose some improving recommendations and hopefully are able to provide those suggestions to the related flight department such as flight training, flight dispatcher and maintenance for future reference and to establish training standard, operation procedures or policies. According to the correlation and importance analysis of this study, the environment and hardware are two factors the most important between those risk factors. This conclusion could then provide for all the relevant flight operation units or individuals who are responsible for the flight operations of his own, such as flight dispatcher, aircraft maintenance, etc. Once those latent or active risk factors have been found out, the existing standard operating procedure, the manufacturer recommended procedures or the related regulations and rules can be used. Those revisions could prevent the occurrence of incidents or accidents in the future. Furthermore, airlines should collect those details as data for the future use when facilitate the induction and training, develop or amend the flight operation policies.
