To estimate the influence of hepatitis C virus (HCV) infection on peripheral blood cell components, and to reveal the correlation between peripheral platelet count, thrombopoietin (TPO) serum levels the HCV infection status among HCV infection blood donors in HCV endemic areas. In addition, the association of HCV viral load and peripheral platelet count was evaluated. METHODS Anti-HCV ELISA positive blood donor samples were tested by RT-PCR for HCV RNA. HCV RNA negative samples were tested by RIBA(Recombinent Immunoblot Assay). Confirmed HCV infection was defined by positive result in either HCV RNA or RIBA. There were 144 confirmed HCV infection blood donors included in the test group and 168 anti-HCV negative donors included in control group. Complete blood count, thrombopoietin serum levels and HCV viral load were measured. A comparison of the TPO serum levels and complete blood count between the two groups was analyzed by Analysis of Covariance (ANCOVA). Correlation between HCV viral load and peripheral platelet count was evaluated by Spearman correlation test. RESULTS Statistical significant high results were found in test group in platelet related items such as platelet distribution width (PDW, p=0.000), mean platelet volume (MPV, p=0.001) and platelet large cell ratio (P-LCR, p=0.000); in red blood cell related items such as red blood cell count(RBC, p=0.038), hemoglobin(Hb, p=0.001), hematocrit (HCT, p=0.002), mean corpuscular hemoglobin(MCH, p=0.027), mean corpuscular hemoglobin conc(MCHC, p=0.004) and in white blood cell related items such as white blood cell count (WBC, p=0.014), monocyte percent(MON%, p=0.000), lymphocyte count (LYM#, p=0.007)and monocyte count(MON#, p=0.000). Statistical significant low results were found in test group in platelet related items such as platelet count (PLT, p=0.000) and plateletcrit (PCT, p=0.000). The mean TPO serum levels in HCV-infected group was higher than negative control group (p=0.000).Peripheral platelet count had a negative correlation with HCV viral load. Peripheral platelet count had a negative correlation with TPO serum levels. HCV viral load had a positive correlation with TPO serum levels (p=0.000). CONCLUSION The findings in this study showed decreased peripheral platelet count, increased red blood cell and white blood cell counts as well as increased TPO serum levels in HCV-infected group. This is the first time that increased RBC was found in HCV infected individuals. The suppression of Peripheral platelet count might be associated with HCV viral load in the blood.
