Evaluation the Effects of Family Empowerment Program on Asthma Symptom, Parental Stress Index and Fmaily Function among Families with Children with Asthma

碩士 === 中國醫藥大學 === 護理學系碩士班 === 98 === Family takes care of children with asthma for a long time,it will increase parental stress and obstruct family function, the high parental stress or obstructing of family function will be also effect children and parents about asthma self management and bodily and mental health. In order to improve the health of children with asthma, reduce of the parental stress and enable family function , the aim of this study was to develop, intervention the asthma family empowerment program(AFEP) based on Freire’s empowerment theory. The AFEP based on the partnership of child-family-clinics to provide counseling, support and enabling family get the capabilities and skills to management their ill child with asthma, improve of parental stress and family function. There are 72 families of children with asthma (ages 6–12 years) have been recruited for the sake of low family function (APGAR < 6) and asthma poor control (ACT< 19) . After randomization by the sealed envelop, thirty four families were assigned into experimental group to receive the family empowerment program, thirty eight families were assigned into the comparison group to receive the regular health education at the asthma clinics. Three-month post intervention have been collected in these two groups for comparison analysis on asthma symptom, lung function, parental stress and fmaily function. The datas were analyzed by mixed model. After received the family empowerment program, Parents in the experimental group have better parental sense of competence and relationship to spouse than in the comparison group after three month intervention (p <0.05), except for the depression and social isolation have significant reduced in the experimental group(p <0.05),however no significant change in parental attachment, restrictions of parental role, parental health , and hyperactivity/distractibility, reinforces parent, emotional, adaptability, acceptability of children between two groups after intervention. About family function, family in the experimental group have better cohesion and expressiveness,except for the conflict and independence have significant reduced in the experimental group(p <0.05). Children in the experimental group have better FEV1 and PEF than children in the comparison group after three month intervention (p <0.05), however no significant change in FEV1/FVC between two groups after intervention. Except for the symptoms during sleep time have no significant change between two groups after intervention; cough, wheezing, and activity limitation have significant reduced in the experimental group. Pediatric nursing emphasizes the family-center. Nurses can play the primary roles as educators in asthma clinics. Use of the expirical results showed AFEP is an important way of enhancing the family’s ability to treat their parental stress, family function,and child’s asthma problems through the cycle of ‘dialogue-reflection-action’. They could cooperation,discussion and dentification the parental stress, family function,and child’s asthma problems in their family, innovative and discovery the way to solving it by the cycle . It is better self-management of asthma than the traditional education.
