The high education puts much emphasis on doing research, which leads to the fact that college faculty attend to doing research and exploring its efficiency whereas they overlook the quality of teaching and providing service. The primary tasks of the university are the dissemination of knowledge and cultivation of future elites and professionals. How to enhance the quality of teaching acts as the key role since our educational policy focuses on the quality of high education. Accordingly, the teaching evaluation has drawn its attention through these years. The ultimate goal of teaching evaluation for college is to promote the quality of both teaching and curriculum, and to give proper responses and feedback so as to help improve teaching quality. With the implementation of The Teaching Excellence Project from the Ministry of Education, each college has paid attention to importance to the assessment of teaching quality, and thereby developed and renewed related mechanism. In cases of teaching-evaluation surveys, the results reveal merely a group of statistic numbers, which might not convey immediately precise information. Hence, a survey system which can provide both directed and proper responses or feedback is comparatively important. Given the lack of thorough feedback system, the present research aimed to build up a feedback model of teaching opinions survey with the application of fuzzy theory and thereby develop an e-feedback system, based on the data resource from National Tainan University. The related information of teaching feedback was sent to each teacher’s email boxes individually via the e-feedback system. The system mainly offers the functions as follows: 1. The rule of evaluation could be decided by the dean of each department. 2. The standard of scores as Functions “Low”, “Medium”, “High” could be decided by the dean of each department, college and school. 3. Provide the results of evaluation for the dean of each department, college and school. 4. Present the overall evaluation scores of each teacher, department, college, and school with statistic graphics. 5. Offer the scores of each front, the average scores, the scores of courses, the comparison of same courses, analysis of items, and the scores of department, college, and the whole school, respectively. 6.
