This study aims to explore the essence of such nurses’ community care competences, in order to enhance their professional competence. The qualitative phenomenological research methodology was adopted. Seven research subjects with schizophrenia were selected via purposive sampling. They had already left a mental recovery section of a mental hospital for continuous community care. The communication record taken during the patients’ continuous care was analyzed by the content analysis method. The results showed that the essence of such nurses’ community care competences include transmitting warmth, helping with self-establishment and providing companionship and adaptation. 1. Transmitting warmth includes:(1)Making the individual enjoy the feeling of being sincerely cared for, treated and respected;(2).making the individual feel that he or she is accepted and understood, and willing to share feelings without being criticized. 2. Helping with self-establishment includes:(1). Leading the patient to explore intrinsic resources inclusive of his or her own self value judgment and leading the patient to perceives self coping ability. (2). Making the patient accept and affirm his or her own abilities, to the extent that, on the basis of his or her past experiences, he or she can experience being recognized, accepted and valued, and can eventually be established and able to manage his or her own health. 3. Providing companionship and making adaptation includes: (1). Forming a accompanied and cooperative partnership with the patient by interaction, altering his or her motivation and making the patient independent investment participation itself to live the adaptation and the health management.(2).Developing coping and adaptation: lead the patient to the question adaptation and utilize, link outer support resources to make his or her own coping and adaptation. It was found that, of the seven cases, one was already steadily on job two were in day-time hospital care, and three could, with the help of their families, manage their own daily schedule while regularly receiving outpatient services. One dropped out of the project for certain reasons. In conclusion, the application of the competence essence for continuous community nursing care was proven to be capable of making patients mentally stable, capable of self-management and fitting in with community social life, and less susceptible to the possibility of rehospitalization. The results of this study might serve as a reference in the continuing care community as regards the practice of nursing staff, capacity development and training.
