The purpose of the study was to investigate the promotion situation of the nursing clinical ladder system in local hospitals; the common and different points of each case hospital; the contextual factors of the system based on individual, leading group and organization levels; and the promotion effects, difficulties as well as responding measures in local hospitals. By purposive sampling, there were eleven clinical nurses chosen from two local hospitals taking part in the research from January to May 2008. The related data were recorded and collected by an intensive interview and then studied by the content analysis. The data analysis and collection were simultaneously conducted. The results showed that the features in common of the individual level were the sense of accomplishment, encouragement, learning development and the improvement of nursing competence, while the differences fell upon the peer interaction and nursing-care ability. In respect of the leading group level, both the common and different points were similar to those of the individual level. The common points of the organization level, however, went to the promotion system, subsidies and group leaders’ cognition, attitudes as well as educational training experiences while the differences of such a level were the operational training regulations and the assistant development mechanism. For the next, some suggestions about how to increase the motive for individual promotion, and how the promotion application and effects could be propelled by directors and organizations were concretely proposed. It is expected through the results of the research to smooth the implementation of the nursing clinical ladder system and to serve in the future as a reference for the related studies on “professional ladder systems”.
