Description
Summary:碩士 === 國立陽明大學 === 護理學系暨研究所 === 96 === Reviews of literature confirm that music therapy may alleviate pain, reduce anxiety, and benefit patient care. There is no associated studies applying music therapy to emergency room. The open wounds are common issues in emergency room, and sutures could cause great anxiety in patients. Reducing anxiety from sutures by music therapy in emergency room was the aim of the study. The subjects with the open wounds received sutures in emergency room underwent randomized sampling. Their level of anxiety was assessed by the anxiety visual analog scale (AVAS), and their heart rates were monitored by non-invasive measurement. The data were analyzed by the Statistics Package for Social Science software (SPSS, 13.0 edition), and the statistical methods used included paired T, independent T, repeated measures ANOVA and Pearson correlation coefficient. P value less than 0.05 was adopted as the statistical significance in this study. The results showed that the average score of anxiety level via visual analog scale (AVAS) of pre-suture management was 4.22±3.03. The age, education level, previous surgical experiences and surgical site were correlated with the anxiety visual analog scale (AVAS) with statistical significance. However, patient gender, arrival time to hospital, accompanied by friends, the mechanism of the injury, the size of the wound did not have any statistical significance. The pre-suture anxiety visual analog scale (AVAS) and the heart rate variation showed positive correlation in Low Frequency Power of music (LF) with statistical significance. There was statistically significant between the pre-suture and post-suture anxiety visual analog scale (AVAS) in paired T test and no statistical difference in independent T test. The heart rate variability between pre-suture and post-suture in music therapy group showed significant difference in paired T test and independent T test. The study suggested that applying music therapy by nursing staff to ease the anxiety of patients during suture in emergency room can be effective. Further randomized control trials with a large sample of subjects with measurable physiological indices were recommended.
