In the health problem of perimenopausal women, sleep disturbances is one of the common chief complaint. However, little studies about sleep quality, fatigue and related factors among perimenopausal women were conducted in Taiwan. The purposes of this study were to explore the sleep quality, fatigue and related factors among perimenopausal women in Gynecologic clinics, and how to these women manage their sleep problems. This study is a cross-sectional, correlation study. Data collected by purpose sampling from 2007 Dec to 2008 Jan at Gynecologic clinics in Taipei City. Eighty-five women aged 42-58 were recruited. The research questionnaires included: (1) background, (2) perimenopausal disturbance scale, (3) perimenopausal fatigue scale, (4) hospital anxiety and depression scale, (5) Chinese version of the Pittsburgh Sleep Quality Index, (6) Self-management and effects of sleep problem questionnaire. The collected data were analyzed through the statistical software, SPSS 15.0. The findings of this study were as follows: 1. The mean age of the women was 52.73. Perimenopausal disturbance was at the level of mild to moderate. Psychological disturbance was the highest level of the perimenopausal disturbance. Women had slightly fatigue to moderate fatigue score. 2. In sleep quality: overall, the total score for sleep quality was 7.71±4.66. 37.6% of women were identified as good sleepers and 62.4% as poor sleepers. 95.3% women were interrupted their sleep during mid-night. The most reason is “getting up to the bathroom during midnight. Women would like to try some methods for their sleep problems such as adjusting their living time, avoiding smoking, drinking and caffeine or arranging the comfortable environment. Finally, they would take the medicine to help their sleep. They found that the medicine was the most helpful for relieving their sleep disturbance. 3. Women who had these characteristics such as “taking hormone”, “taking plant hormone”, “taking medicine long term” and “being in perimenopause”would have worse sleep quality than women had none of these characteristics.(p<.05) Sleep quality had significant positive correlated with perimenopausal disturbance, perimenopause fatigue, anxiety and depression 4. The “take medicine long term”, “take hormone”, “take a plant hormone”, “climacteric status”, “perimenopausal disturbance”, “perimenopausal fatigue”, “anxiety” and “depression” explained 52.9% of the variance of sleep quality. The predictable variables were “perimenopausal fatigue” and “depression”; they can explain 39.5% and 5.5% of the variance of the satisfaction of sleep quality. The results of study, offer the important reference about sleep quality of perimenopausal women. Health providers should pay attention sleeping health problems for women who had these characteristic:“taking hormone”, “taking a plant hormone”, ”having nocturia”, “taking medicine long term”, “being in perimenopause”, “fatigue” and “anxiety or depression”. In addition, perimenopausal sleep consult and sleep hygiene health education should include these programs: how to self-manage the sleep disturbance, decrease fatigue, and relieve the anxiety and depression by seeking support system, as well as offering health information about taking hormone therapy, taking nutritional supplements and the safety medicine.
