To reduce the risk or slow the progression of complications, optimize blood sugar, blood pressure, and blood lipid. Patients were asked to receive exams regularly. How can we use information technology to improve diabetes care become an important issue. In this study, we propose a diabetes complication risk factors long-term monitor system to provide physician more information for the purpose of better health care. We follow the latest practice guideline, calculator risk factor and present long term examinant data for different complications. Moreover, we combine current hospital information system, offer quality assessment tools to physicians. To consider the information safety and system stably, we did not test our system online. We also demonstrate that our system is able to provide necessary functions to physicians
