Coronary heart disease and stroke are the two most critical types of the cardiovascular disease. For example, in Taiwan, there are more than 43,000 people died of cardiovascular disease over 2003. The cost of cardiovascular disease is also very expansive. Therefore to promote the development of novel prevention and care approaches in cardiovascular disease is an important research train. The absolute risk of cardiovascular disease in any individual can be determined by a complex interplay of several factors. There are many evidences show that continuously improving these risk factors of cardiovascular disease can considerably reduce cardiovascular morbidity and mortality and then obtain a better life quality. To many cardiovascular disease patients, it is short of an alter warning in the early phase. In this thesis, we proposed a web-based approach to self assessing and monitoring the cardiovascular disease risk factors. There are two main objectives of this development: First, to help the people to assess their risk of cardiovascular disease based on the evidence-based medicine, and educate the people with the knowledge of risk factors of cardiovascular disease; second, to collect the patient’s health information for a period of time, and then facilitate the physicians to take follow-up of the cardiac patients. In order to comply with our purposes, we exploit the software agent technology in our system that build-in the assessment rules based on evidence-based medicine and follow the FIPA (Foundation for intelligent Physical Agents) standard. The agents are in charge of the following tasks: to estimate the risk of cardiovascular disease for patients, to find the suitable health education information based on the patients’ need, to manage the patients’ health information, to make decision of informing the physicians if the patient’s situation has wildly varieties, and so on. This system is implemented on a web-based application; therefore the people can use it anytime and anywhere.
