The relationship between these variables is also identified. The research is carried out based on questionnaires. The subjects of the study are teachers from elementary school special education classes and resource classes in Changhua County. These questionnaires were designed by the researcher as a research tool for reflecting on current practices, needs regarding the implementation of elementary transition. The valid questionnaires retrieved are analyzed by frequency distribution, percentage, mean, and standard deviation. The results are shown as follows: I. The current situation of establishing individualized transition plan of special education teachers. a. Most teachers hold an annual ITP meeting one year before the students graduate from elementary schools. The average time allocated to each student is between 11 to 30 minutes. b. Most teachers have written ITP for students and include it within the IEP. c. Teachers wrote ITP based on the opinions of the parents mainly. d. The most difficult problem special education teachers face when dealing with transition actions is the lack of human resources. II. The implementation of special education teacher transition practice in Changhua has reached partly carried out levels with respect to administration, counseling for advancing to higher schools, counseling for life management, psychiatric counseling, and welfare services and related professional services. Among them, administration ranks the highest for its outcome. III. The earnest need of special education teachers in Changhua is the help from educational institutions to offer ITP samples for reference. IV. The relations between backgrounds of teachers and the implementation of transition a. The value of correlation is the highest between schools of different sizes and administrative practice. The value reaches -.223. b. There is a negative correlation between teachers of different backgrounds and transfer needs.
