Summary: | 碩士 === 國防醫學院 === 牙醫科學研究所 === 95 === Full mouth reconstruction under general anesthesia is not only an important treatment option, but also often considered as the last choices in pediatric dentistry. It helps children who are not able to cooperate, whether emotionally or physically (i.e. handicaps) with dental treatment. With social-economical progress and the guidance of government health department, the reduction of dental caries rate had become the main goal of preventive dentistry in Taiwan. The work of reduction dental caries prevalence needs a period of time and a large group sample. However, it is interesting to find that in dental clinic, the age of patients who suffer from dental caries keeps dropping down. The uncooperative young patients are the major headache for both the health care workers and the parents. Unfortunately, the relating case reports and studies are limited. The goal of this study is to cross-examine the treatment details and the factors may affect the treatment procedure. The fist part of the study is the data collection, which includes the patients who received full mouth reconstruction under general anesthesia from March 2001 to March 2007 in TSGH. The total patient number is 186. Statistical analysis were performed on variables among the year of operation carried out, patient’s age, gender, living places, physical conditions, treatment contents, anesthesia medications and the duration of hospitalization. These patients’ recall conditions are recorded and discussed as the second part of this study. The third part of study focuses on analysis of the
questionnaires, which were collected from the 94 out of the 186 patients treated under general anesthetics as in the first part. The contents of the questionnaires include the oral health, physical health, psychological health and oral health care behavior. In the end, the results from part one to part three are pooled-up, cross-examined and analyzed with statistic. The following results were obtained: 1. In ECC group ,the average ages of the patients who seek for dental treatment were decreasing over the studied years. 2. Living place: primarily in Taipei city (90%). 3. Distribution of gender: the younger group is male-predominant, whereas the older group is female-predominant. 4. Duration of hospitalization: positively related to the handicapped patients. 5. Caries severity index: the early child caries (ECC) group is worse than the handicapped group. 6. The anesthesia recovery time: longer in the handicapped group than that in the ECC group. It is also positively related to body weight, the gender (male), and negatively related to the anesthesia application time. 7. The surgery time is positively related to amalgam restoration, pulpotomy and pulpectomy. 8. Post-operation fever is negatively related to age.Mental problems of children and adolescents will affect the patients’ abilities of keeping oral hygiene, and cooperate with dental treatment. It is a valuable task to make an early diagnosis and establish a referral system through regular dental check-up to improve patients’ oral health.
