| Summary: | 碩士 === 國立成功大學 === 政治經濟學研究所 === 95 === The National Health Insurance initiated by government in 1995 enables all nationals to have an equal chance to join and to meet the demand of health. As a whole, the policy on NHI not only influences the standard of living deeply, but has been paid attention and discussed in public on the issue that focuses on state-run business with turnover more than 300 billions for a period of time. The enforcement of NHI system is to add those below 14 and above 65 without the protection of health care to the umbrella of health insurance system. According to this, the policy of NHI ranging from the population to social resources is one policy that have a great impact on government.About 65% are satisfied with NHI in the beginning of initiation, with up to 80% satisfied in late 2004. But it can’t be denied that being highly satisfied with NHI isn’t good enough to represent great effectiveness. On the contrary, the system of NHI still has some space to reform elsewhere.Practically, any sector such as medical field or government all thought that the enforcement of NHI which government initiated is on a rush way under pressure. Hence, many policies and system’s enforcement about related measure failed to have consensus with society without consecutive advertisement in public and resulted in financial crisis. So it depends on the stability of finance to see if the NHI can run everlastingly. And how to save NHI to achieve the balance of finance for government is becoming the first topic on medical implement. On the other hand, the ratio of medicine expenditure among high medical expense is the first major cause that resulted in the rise on expenditure of medical expense.
This research with the new institutionalism analyze the development of National Health Insurance,Separation of Prescribing & Dispensing and National Health Insurance payment system. In 1997 the SPD policy implement, concerning pay institutional reform to then include macroscopic , Global Budget System adoption and tiny view , is carried out effort fee-for-service payment system to DRG payment system.
