There is much uncertainty on construction sites, comprehensive inspection is the most important step to ensure the safety of the workers. But the shear number of scaffolds used in a construction site poses a challenge for the inspecters. A detail examination requires a long time to complete. And the detail of the examination sometimes depends on the experience of the examiner. An effective and simple check list style inspection form can be a useful tool for the inspecter. It can pick out the scaffold system with the highest potential of disaster. It can also raise the safety standard by increasing the number of scaffold systems receiving inspecttion. Both can be helpful in reducing the possibility of occupational incidents. The current research investigates the causes of scaffold related incidents. Factors that reduce the loading capacity of the scaffold system and the failure modes of the scaffold system are studied. Numerical simulation is performed to explore the effect of these factors. After a thorough understanding of the effects of these factors on the loading capacity of the scaffold system, a check list style inspection form is proposed.
