Most hospital adopted information technology to a certain level to adjust the frequently changes of healthcare reimbursement policies. The hospital information systems have extended to clinic procedures recording, on-line examination facility, digital imaging storage, etc. However, from the management perspective, there should be a standard format or principle to achieve an open environment for medical informatics development to reduce errors and mistakes while exchanging information. Most prior studies related to PACS were focused on the implementation or the users’ acceptance of PACS technology and few studies were in the area of integrating the healthcare enterprise (IHE). Recently, large-sized hospitals emphasize in the development of PACS. The purpose of IHE was to solve the problems caused by different modality among different medical information system and achieve information sharing. Therefore, this study interviewed doctors and specialist of the radiology department and members in the information system department of four case hospitals to achieve the following goals: first, to identify factors of concern about standard such as HL7, DICOM3, and IHE and secondly, to indicate the changes of work flow caused by using IHE while adopting PACS. The results of this study can explore the acceptance of IHE of the case hospitals, provide information to understand the resistance or assistance of adopting PACE, and serve as a reference for other hospitals while adopting PACE to improve the quality of health services and benefit the publics.
