碩士 === 國立高雄第一科技大學 === 風險管理與保險所 === 94 === ABSTRACT Health services utilization of insured object and the variation of health care seeking behavior not only need to control the financial expense of health insurance that is aim at the aspect of medical treatment demand, but also need to restrain the medical resource inappropriate use in the aspect of supply. This text would like to probe the influence of western medicine ambulatory care system. Therefore, the research period of observation system comes into effect that is during Jan. 1st of 2003 to June of 2003One of the fluctuant time points is on July of 2002 which implements western medicine hospital global expense system. Another fluctuant time point is on Sep. of 2002 that adjusts different level referral ambulatory care co-payment system. The other fluctuant time point is on Apr. of 2003, because this text research period involved SARS separation defense, and also to observe SARS period that whether have possibility to restrain health services utilization or not. In this text, combine four period and analysis the different influence of long-term and short-term in order to understand exactly the change before and after of every risk cluster.Therefore, this research method can be divided into three kinds of observation types. Most of documents of health insurance policy adjusted give influence of medicine services utilization are the static analysis of statistics, but this research analysis by Two-level self-organizing maps, it observes the dynamic tendency. First, according to first-level characteristic to analysis that risk level cluster of health services utilization. Second, through second-level discuss the dynamic tendency. Lastly, combine trajectory maps and dynamic tendency chart to go a step further to observe the factors of health situation. And subsume Adjusted Clinical Groups into the reason of seeking medical treatment behavior alteration and analysis it.
