Summary: | 碩士 === 國立中興大學 === 獸醫學系暨研究所 === 94 === The Complication after the cataract surgery, around 25% to 45% relates to the corneal pathological change. In the procedure of the cataract surgery, both topical disinfectant and intraocular irrigative solution are contact with the cornea directly. In this study, different kinds of topical disinfectant and intraocular irrigative solution were used on rabbit eyeballs to compare the changes of the rabbit corneas post- operations. 32 eyeballs belonge to 16 female New Zealand White Rabbits, all the rabbits are 6 to 7 months old. These eyeballs were divided into 4 groups with 8 eyeballs in each . One was used as the control group without using any intraocular irrigative solution; whereas other three groups used different kind of intraocular irrigative solutions- 0.9% NaCl Solution, Ringer´s Solution and BSS Plus respectively. For those groups used the intraocular irrigative solutions, half of the eyeballs in each group used Providone-Iodine 1: 200 and Gentamicin 3mg/ml as the topical disinfectant. The time used for both sterilization on the eyeball surfaces and the irrigation were controlled at 5 minutes in each test. The basic optical test includes intraocular pressure (IOP) , Schirmer Tear Test (STT) and corneal fureolscein stain test were conducted pre and post operatively on immediately and day 14. The Statistics for Cornea Thickness (T), Corneal Endothelial Cell Density (CD), and corneal endothelial cell morphometric analysis were taken pre and post operatively immediately, day 1,day 2,day 3 and day 14. 14 days after the operation, for those rabbit eyeballs used Providone-Iodine to sterilize, the STT records were reduced by 15.89%; whereas for those used Gentamicin 3mg/ml the STT records were down by 13.64%. The differences were not significant. However, the eyeballs sterilized by Providone-Iodine 1:200 caused superficial corneal injury, hence the cloudness on the corneal epithelium. Nevertheless, this symptom disappeared within 14 days after the operation.
The Corneal Thickness was most severe for those eyeballs used Ringer´s Solution, 1 day post operative, the Cornea Thickness statistic was increased by 10.44%, followed by those used 0.9% NaCl Solution at 7.31% 2 days post operative and BBS Plus at 2.23% 2 days post operative. The Corneal Thickness for these three groups returned to normal status at 14 days post operative. The greatest reduction on Corneal Endothelial Cell Density belongs to the group used the 0.9% NaCl Solution, the Cell Density reduced 8.33% 1 day post operative. Moreover, the group used the Ringer´s Solution reduced the Cell Density by 5.31% 2 days post operative. However, the Cell Density in the group used BSS Plus remained steady. 14 days after the operation, the Cell Density in the group used Ringer´s Solution returned to the normal status; whereas the Cell Density in the group used 0.9% NaCl Solution remained 7.21% in reduction. It was easily to conclude the damage in Cell Density was the greatest for those groups used the 0.9% NaCl Solution and at the same time the damage to the morphometric type was the most critical ones. These study brought to a result to conclude: Use the Gentamicin 3mg/ml as the eye ball topic disinfectant, the damage brought to cornea was far smaller than the traditional disinfectant Providone-Iodine 1: 200. Amongst the intraocular irrigative solutions being used in these experiments, the BSS Plus represented the smallest change in terms of both the Corneal Thickness and Pathological Changes to corneal endothelium. We could easily conclude the BBS Plus is the most outstanding intraocular irrigative solution available nowadays. By using the best available eye ball topic disinfectant and intraocular irrigative solution, together with the superior operation techniques, the successful rate for the optical operation could be improved and reduced the possibility for complication to occur after the optical operation.
