General consideration and clinical application of periodontal prosthesis in periodontitis cases

碩士 === 高雄醫學大學 === 牙醫學研究所碩士在職專班 === 94 === In 1974, Dr. Amsterdam, the professor of Pennsylvania University, defined “periodontal prosthesis” as the restoration and prosthesis that are essential during the treatment of patients with severe periodontitis. Besides, the concept, principles, technique of periodontal-prosthetic treatment is also applicable to utilize in the restoration and prosthesis of natural dentition. The periodontal-prosthetic treatment is a comprehensive and planned rehabilitation of the oral cavity which combines with the endodontic, restorative, orthodontic, periodontal, and prosthodontic treatment. The factor of successful treatment of periodontal prosthesis is dependent on choosing a suitable human nature treatment plan, which builds on the basis of good communication and cooperation between the patients and the operators, and the consideration of the financial ability, and personality of the patients. The patients with severe periodontitis could also result in loss of alveolar bone, deep periodontal pockets, and more seriously, the eccentric occlusion, collapse of posterior teeth and tempromandibular joint disorder due to greater tooth mobility and dislocation of the teeth. In our reports we presented the oral rehabilitation procedures of 9 cases with moderate to severe periodontitis, including periodontal and prosthetic therapy. Periodontal treatment included guide tissue regeneration technique by using different biomaterials, the crown lengthening procedure, connective tissue graft, and sinus lift; the final prostheses included fixed or removable prostheses designed by conventional, implant-supported, and extracoronal resilient attachements apparatus. In the routine maintenance, all patients maintenanced healthy periodontal condition with good oral hygiene control and were satisified with the restoration of occlusal function, pronunciation, and esthetics. The dentist haves to make treatment plan decision combined periodontal therapy with prosthetic design according to not only periodontal condition, but also the personal demand, and financial capabilities of the patients to achieve most appropriate treatment result.
