Although the modern medical technology is so thriving, so far by now we still can not manage to find a substantial cure to it. We can only rely on long-term medication control to prevent it from getting worse and lessen the complication effect. In November 2001, the Bureau of National Health Insurance introduced “「全民健康保險糖尿病醫療給付改善試辦方案」” and in March 2004 the Bureau of National Health Insurance promoted “「全民健康保險家庭醫師整合性照護制度」”. These two schemes carried out by the Bureau of National Health Insurance are based on a same objective, to establish a patient-oriented medical care system in the meantime building up health insurance and quality of life concept and ultimately, to elevate the medical care service quality. This research is to find out the impact of diabetes patients involvement on disease management and the difference of Process Indicator and Outcome Indicator on diabetes patients involvement. The result showed that, 1.The Process Indicator (diabetes knowledge, diabetes behavior control, service satisfaction ) showed a significant difference between before and after management. Cases over one year, over six months and un-closed cases showed a notable difference in diabetes knowledge, diabetes behavior control and service satisfaction. 2.The Outcome Indicator was no significant difference. In the study period, the patients who received the disease management, the Result Index (fasting blood sugar level, HbA1C level and cholesterol) had significant improvement. The three groups did not have much difference in Result Index, but if we go on further analyze and focus on cases less than one year and cases less than six months, The Result Index has dramatically fall on fasting blood sugar level, HbA1C level and cholesterol. Hence, we can conclude that disease management’s involvement on diabetes can level up the quality of diabetes patients’ medical treatment. It is a scheme worth a nation –wide promotion.
