Study of Timing and Dosage about Shen-Hua-Tang on the Postpartum by Delphi Technique in Taiwan

碩士 === 長庚大學 === 傳統中國醫學研究所 === 93 === ABSTRACT Sheng-Hua-Tang is commonly used for the postpartum recovery in Taiwan. The purpose of this study was to understand the differences about the dosage, composition and initial timing of Sheng-Hua-Tang on the normal spontaneous delivery（NSD）, cesarean section（C/S）and artificial abortion（AA）. We discussed the influence of physician’s background on the prescription of Sheng-Hua-Tang. The point of this study was also to realize the appropriate dosage, composition and initial timing of Sheng-Hua-Tang for the postpartum recovery by Delphi technique. 34 gynecologic experts in traditional Chinese medicine (TCM) who have ever taught in the China medical college, practiced in the teaching hospital or recommended by the fellows from TCM gynecologic committee were enrolled in this study. The semi-open questionnaire was used to survey for the purpose of this study. The results revealed that the dosage ratio of Radix Angelicae Sinensis and durations of decoction prescribed for NSD were higher than C/S. The dosage ratio of Radix Angelicae Sinensis, Rhizoma Ligustici Chuanxiong and durations of decoction prescribed for AA were lower than NSD. The initial timing in taking Sheng-Hua-Tang was longer than 24 hours after delivery from the opinions of most experts. Sheng-Hua-Tang needed additional drugs when it was prescribed for postpartum recovery of the patient. It was not necessary to add any drug for breast-feeding mothers. About the preparation of Sheng-Hua-Tang, there was no special requirement for NSD and AA. It needed extracting with pure water for C/S. About the impacts of physician’s background, we found that there were significant differences in sex to the durations of decoction prescribed for C/S, license resource to the dosage ratio of Radix Glycyrrhizae Praeparata for NSD, drug adverse effect of Sheng-Hua-Tang to the dosage ratio of Semen Persicae for NSD and Rhizoma Zingiberis Praeparata for AA. The drug adverse effect of Sheng-Hua-Tang was an important factor that influenced additional drugs when prescribed for C/S and AA. The license resource and whether having western medical specialist license or not influenced additional drugs when prescribed for breast-feeding mothers of NSD. The license resource, whether having western medical specialist license or not and drug adverse effect of Sheng-Hua-Tang, they all affected additional drugs when prescribed for breast-feeding mothers of C/S. Drug adverse effect of Sheng-Hua-Tang and license resource influenced the preparation of Sheng-Hua-Tang for AA. The initial timing in taking Sheng-Hua-Tang for NSD was affected by whether having western medical specialist license or not. In conclusion, after two rounds of questionnaires by Delphi technique, the dosages of Sheng-Hua-Tang for NSD were Radix Angelicae Sinensis 4.75～8 CN、 Rhizoma Ligustici Chuanxiong 2～4 CN、Semen Persicae 1 CN、Rhizoma Zingiberis Praeparata 0.5～1 CN、Radix Glycyrrhizae Praeparata 0.65～1.5 CN and the durations of decoction were 7～9 days. The dosages of Sheng-Hua-Tang for C/S were Radix Angelicae Sinensis 4～6 CN、 Rhizoma Ligustici Chuanxiong 2～4 CN、Semen Persicae 0.95～1CN、Rhizoma Zingiberis Praeparata 0.5～1 CN、Radix Glycyrrhizae Praeparata 0. 5～1.25 CN and the durations of decoction were 5～7 days. The dosages of Sheng-Hua-Tang for AA were Radix Angelicae Sinensis 4～7.25 CN、 Rhizoma Ligustici Chuanxiong 2～3 CN、Semen Persicae 1 CN、Rhizoma Zingiberis Praeparata 0.5～1 CN、Radix Glycyrrhizae Praeparata 0. 5～1 CN and the durations of decoction were 3～6 days. It was necessary to have additional drugs when prescribed for postpartum recovery. No additional drugs were needed for breast-feeding. About the preparation of Sheng-Hua-Tang, there were no special requirements for NSD and AA, except extracting with pure water for C/S. The initial timing in taking Sheng-Hua-Tang was longer than 24 hours after delivery for NSD and AA. Most opinions about the initial timing in taking Sheng-Hua-Tang for C/S were the time after flatus passage. Key Words：postpartum recovery，Sheng-Hua-Tang，Delphi technique，traditional Chinese medicine
