Summary:碩士 === 國立雲林科技大學 === 企業管理系碩士班 === 92 === Suffered from official budget and personnel expenditure constrained year by year and the implement of global budget imposed by BNHI, governmental hospitals are encountering the difficulty to maintain their financial balance. Hospitals of DOH, originated from previous provincial hospitals, were forced to be self-supporting since July of 1999. All these hospitals are facing being down-sized or merged by private financial organizations or hospitals of medical centers if the performance is not good enough. Experienced the above example, governmental hospitals, which were used to receive government financial support, have to change their management way for long term business. Superintendents of these hospital’s chief have to meet this great challenge and take their responsibility... The main purpose of this study is to confirm the interrelation between the structure reform by governmental hospitals due to environmental change, and Total quality management to improve organizational performance; to find the relevancy of the strategy taken and the resistance occurred during reforming process, and the reform action; to seek the relation of the applying reform activities to the outcome; to define whether there is relative between positive/negative influential factors of the reformation and hospital efficacy,and this study is explose of importance problems,too. This study is based on quality method: mainly on literatures, collected and edited real data and actual visiting, and secondarily on the questionnaires provided by the target hospital. All materials were described and analyzed for finding the interrelationship, expected result and proposals of this study, and looking forward for giving reference to those hospitals that are implementing or are going to implement structure reform, and providing the conception of promoting and planning structure reform for superior medical organizations of DOH’s hospitals.
