Summary: | 碩士 === 慈濟大學 === 護理學研究所 === 92 === The purpose of this study was to develop an evaluation tool of orthopedic nursing care quality and to evaluate the quality of nursing care. The research started on Sep.1, 2003 and ended on May.31, 2004. Random number table was used to select all orthopedic patients randomly from two orthopedic units in the teaching hospital at eastern region of Taiwan, a total of 180 orthopedic patients were recruited. Research subjects were classified into three different types according to modified Proto Type Patient Classification System. A standard questionnaire was used to evaluate the quality of nursing care. Nursing care evaluation tools were modified from Orthopedic Nursing Care Quality evaluation tools developed by Hsu (2002). The standards of nursing care were based on the patients’needs and the nursing process. There were four main objectives: (1) the plan of nursing care was formulated, (2) the physical needs of the patient were attended, (3) the psycho-social-cultural-spiritual needs of the patient were attended, (4). achievement of nursing care objectives was evaluated There were many sub-objectives, standards, and criteria under the main objectives. The Inter-rater reliability was 98%;The intra-rater reliability was 100%. The content validity was evaluated by five experts, the value of CVI is 0.97.The result showed the mean scores of four main objectives were 85.91, 85.84, 80.11, and 89.47.Records document the care provided for the patient(97.58),and the patient is protected from accident and injury(95.80)had the highest scores. However there were low scores on the need for skin care was attended(8.16),the need for psychological-emotional well-being was attended(39.94),the need for respiratory care was attended(59.55). The mean scores of four main objectives in Orthopedic Unit A and Orthopedic Unit B were 87.13 and 85.64, 85.97 and 85.96, 79.79 and 78.79, 89.54 and 89.36. The research also found the priority in achievement of nursing care objectives and the plan of nursing care were higher than the physical needs of the patient and the psycho-social-culture-spiritual needs of the patient. To help the hospital and nursing administrators to improve the quality of nursing care and clinical teaching quality, according to the results of these study 14 suggestions were recommended by the researcher.
