Summary: | 碩士 === 國立台北護理學院 === 護理研究所 === 92 === The purpose of this study was to construct a theory on the process of disease cognition, especially focusing on the elderly with type 2 diabetes who is facing the insulin therapy. We use the “Grounded theory” as our main research method. It was a deep and private talk which took place in a diabetes education center within a regional hospital. We interviewed 9 elderly persons with type 2 Diabetes, whose age were 55 or above ranging from 55 to 85 years old, treated regularly at the diabetes clinic. We found that the「will to survive」was the core category of the disease cognition process, it drove the whole process.「Obstacle of previous health problems」was the leading period, when a patient encountered the health problems; he or she would seek for the proper survival skill. There were three key elements in this process---「motivational power」,「thinking disease and me」, and「modifying lifestyle」.「Motivational power」was the most important element of the「will to survive」; it could help and support the patients through different stage of the health problem.「Thinking disease and me」 might include some different aspect of viewpoints such as 『inner feeling of the disease suffering』,『temptation of taking action』and『explanation of the disease』.「Modifying lifestyle」 included the『choice of new life』,『thinking the need of life』,『construction of the knowledge and skill』, accumulation of experiences』and 『building the regular livings』. These would find the final solution of the「living together」. There were many factors influencing the disease recognition process, for example:「medical resources」,「interpersonal relationship」,「health status」, and「social cultural values」. From our study, it can form the basic construct of the disease cognition process of the elderly with type 2 diabetes who is facing the insulin therapy and teach the patients to cope with the problems through different stage of the cognition process; thus would improve self-care actively and positively.
