碩士 === 國立高雄第一科技大學 === 環境與安全衛生工程所 === 92 === Reviewing the localized safety management system which has been established on the basis of regulation of safety criteria, it is a very systematic process either in its structure of regulation or compliance surveillance. But the governmental statistic data showed that the total thousand incident rate is still high up to 4.8~7.1 during the past ten years in manufacturing process industries in Taiwan, the effectiveness and consistency of the safety management system against to reduction of occupational disaster is doubt obviously. and the Near Miss Reporting system is one of the major weakness in the local safety management system, In accordance with the foreign literature report showed that Near Miss Reporting will minimize the further accident or personal injury and property damage significantly. The safety pyramid or iceberg theory (Heinrich 1931) & (Bird 1966) indicated that to take off the bottom near miss incident element will minimize the top event or incident occurrence , The Domino effect of root causation will be prevented eventually. This is coincided with the ancient Chinese proverb said that the three inches thickness of ice was not derived from the water within a day. It is a evident of Murpher’s law that something become worse it will be wrong , in another word that if we ignore the near miss incident then the real accident will be happened to us. This primary study is aim to research the safety management based on Near Misses Reporting implementation within the process industries in Taiwan and try to build up a localized Near Miss Reporting model for local process industries requirement . It will improve occupational disaster under appropriate implementation. Two evidences of effectiveness and available to safety management has been verified by two case study of near misses reporting in the local process industry. .
