This research used the 35 department of healthcare (DOH) hospitals as case to study the critical success factors of developing and spreading the strategic alliance information system (SAIS). This study references Ulrich and Barney’s resources dependency, Davis’s technology acceptance model, Porter’s cost advantage, and D&M’s information system success models. Using modified Delphi method, a structured questionnaire with five-point Likert scale is adopted to collect expert’s opinions. The experts included twelve superintendents and eight CIO of DOH hospitals. The Delphi results summarized five classes and seventeen propositions as the critical success factors of developing the SAIS. Cost advantage is the most influential factor. Factors with average scores greater than four are considered as critical success ones. They are economic scale, budget controlling, cost sharing, competition in same business, interaction relationship, top management support, government policy, decreasing government budgeting support, and flexible work time. The critical success factors of SAIS are difference from those of general information systems. General information systems are influenced by system technology greatly. For example, in D&M information success model, the intention to use and user satisfaction are influenced by information quality, system quality, and service quality. The SAIS is influenced mainly by cost advantage. Cost advantage includes economic scale, interaction relationship, cost sharing, and flexible work time that bring significant benefits to alliance members. Through developing the SAIS, members can lower down their cost and enhance their competitiveness.
