Summary: | 碩士 === 國立陽明大學 === 衛生福利研究所 === 91 === The purpose of this study was to explore the impact of psychosocial problem and intervention on the outcome of the inpatient with schizophrenia.
Secondary data was obtained from Multidimensional Psychopathological Group Research Program (MPGRP) leaded by professor Hwu,Hai-Gwo. The original interview was conducted by doctors or qualified trainers, using a structured questionnaire. 225 subjects were selected from Part I of the questionnaire. Psychosocial problems referred to psychological conflict, family conflict, social role dysfunction, interpersonal relationship and life problems. Psychosocial interventions referred to individual psychotherapy, group psychotherapy, family therapy, and social skill training and life arrangement.
Data was first cleaned and checked for consistency, and then analyzed by SPSS. Chi square, t-test, ANOVA and GEE (Generalized Estimating Equations) were the major statistical analysis applied in the study.
The results of the study were as follows:
1. More than 85% of the schizophrenic inpatients had psychosocial problems, 58% of them have social role dysfunction and 54% of them have life problems.
2. The most common psychosocial interventions at ward were social role therapy and life arrangement.
3. Patients who had psychosocial problems had poor social functioning, after 3-year follow-up, than those who do not have. Those who had received life arrangement consultation during hospitalization and those who took medication regularly were less likely to relapse.
4. Those who received psychosocial intervention or those who had social support had better social role function than those who do not.
5. No significant statistical relationship was found between relatives’ expressed emotion (EE) and schizophrenic relapse.
6. In sum, the following factors had significant impact on patients’ outcomes during three year’ follow-up: patient’s marital status, working status, age at onset, duration of the disease, type of the disease, negative symptoms, the attitude of cooperation, drug compliance and psychological conflict.
The result of this study can help the psychiatric professionals to understand the importance of psychosocial problems and the need for psychosocial interventions of the schizophrenic patients during hospitalization to improve their quality of life.
