Sixty mothers who had very low birthweight babies with complications were chosen from the neonatal intensive care unit and newborn center of a medical center in Taipei and assigned to a treatment group or control group in equal numbers. Individual home care supervision was provided to mothers in the treatment group after their babies were discharged from the hospital. Mothers in the control group were provided home care supervision after the end of the study. The effects of improving the level of knowledge and reducing the degree of anxiety of the mothers of very low birth weight babies with complications on the quality of infant care was evaluated. In addition, the relationship between knowledge and anxiety of the mothers was assessed to serve as a reference for developing home care supervision guidelines. The mothers in both the treatment and control groups completed a questionnaire before discharge from the hospital and again one week after leaving the hospital. The software package SPSS/Windon10.0 was chosen for statistical analysis of the data. No significant differences between the two groups were found in the characteristics of the very low birthweight babies, the mothers, or their family traits. Although no significant difference existed in the knowledge of the mothers in the two groups before being discharged from the hospital, mothers in the treatment group experienced a significant increase in infant care knowledge after receiving home care supervision compared to the slight increase that occurred in control group. An obvious reduction of anxiety in the treatment group occurred after undergoing home care supervision home compared to the control group, even though there was no difference in the anxiety level of the two groups before studying and a greater number of mothers in the treatment group than in the control group were anxious prior to the study. Factors that influenced the degree of a mother''s anxiety included the gender of the child, the type of gastrointestinal complication, the mother''s job status before delivery, and the father''s age. Factors that affected a mother''s home care knowledge included the mother''s job status before delivery, her religious beliefs, and the amount of information she read about prematurity prior to delivery. Results on the relationship between home care knowledge and anxiety of the mothers indicated that the lower the mother''s home care knowledge before the study, the higher her anxiety level; and the more knowledge provided to a mother through home care supervision, the lower her anxiety level. The results of this research increase the understanding of the effect of home care supervision on a mother''s infant care knowledge and anxiety about caring for a very low birthweight baby with complications. This can serve as a reference to other mothers who have very low birthweight babies with complications.
