The processes would increase tissue mortality and is called “ischemia/reperfusion injury” (I/R injury). If the I/R injury become advanced, the “no-reflow phenomenon” of the capillary bed will eventually lead to the necrosis of grafts. It is important to reduce the I/R injury during transplantation. The purpose of this study was to evaluate the damage of rat free vascularized flap resulted from I/R injury, through observing flap superficial blood flow, population of infiltrating neutrophils, levels of the interleukin-2 (IL-2) and interferon-gamma (IFN-γ) mRNA in the skin tissue, and flaps viability. Lactated Ringer’s solution (LR solution) was chosen as the perfusate in this study. An epigastric free flap transfer model with microvascular anastomosis was used. All animals were observed for one week postoperatively to assess flap viability. On skin flaps suffered from different periods of ischemia, laser doppler flowmeter was used to detect the blood flow in the transplanted skin. Histological structure of the skin tissues were also evaluated. Skin biopsy was taken at the 1st and 7th days post-reperfusion and/or when flap necrosed. Biopsies were either fixed in 10% buffered formalin for H&E staining or stored at — 80 ℃ for cytokines analysis. With ischemia duration prolonged, increased amount of infiltrating neutrophils and decreased flap superficial blood flow were found. Long ischemic time may lead to necrosis of skin flaps. The level of IL-2 and IFN-γ mRNA in the 7-hrs-ischemia group were both significantly increased at the 7th day. In the 12-hrs-ischemia group, all flaps necrosed at the 2nd day, the IFN-γ level at the 2nd day was significantly higher than that at the lst day. With prolonged ischemia duration, the damage to the skin flap caused by I/R injury was more advanced.
