Summary: | 碩士 === 國立台北護理學院 === 長期照護研究所 === 91 === Victim with chronic conditions increases gradually as a role. And the number of tube-fed elderly is lots enough to call attention of publicity. However little researches aimed at nutritional status of this population. Since malnutrition endanger the health has been well documented; we try to explore the nutritional status and impact factors of tube-fed elderly in the long-term care facilities. The elderly was divided into 3group as with natural food group (N), mixed-typed food group (M) and formula-typed food(F), by arbitrary sampling. The related biomarker including serum albumin, total cholesterol, hemoglobin and anthropometrics indexes including BMI, calf circumference, waist circumference and sagittal abdominal diameter were measured as the nutritional indicators. Three-day diet diary was traced to screen the deference of nutrients among the three groups. The Mini Nutritional Assessment (MNA) scale was applied. The results showed 19 of N group , 22 of M group and 70 of F group., totally 111 subjects were examined. MNA<17, malnutrition, was noted in 57﹪of the study population. Divided according to feeding type , the MNA score were <17 in 100﹪of individuals of both N and M , 30.8﹪of F group. The average caloric intake were deference significantly(p<0.001).Both N and M were fed under the Basal Energy Expenditure(BEE), F group were over the BEE contrarily. The caloric density of natural food was 0.66 and 0.4per gram respectively in N and Mgroup respectively The subjects under care by dietitian showed better on total cholesterol, hemoglobin(p<0.05)and anthropometrics(p<0.001).The risk factors of nutritional status were number of tube inserted , PVC material of feeding tube, medication used over 9, long period of constipation, denial care by dietitian and receiving natural food , noted by logistic multiple regression .ROC curve was also used in order to make an easy indicator for anthropometric assessment. Concerning for nutritional support by administrator, reasonable number of nursing staff and dietitian are required to promote the nutritional status of institutionalized ageing people..
