Empirical evidences showed bi-polar development of healthcare industry. Large hospitals became larger, and clinics went prosperous too. Yet the number of area hospitals decreased. 　　The scale of most public hospitals are district or area based. The operation of public hospitals greatly depends on governmental subsidary. When governmental finance viability gets worse, the challenge public hospitals face is more difficult than that of private ones. 　　The service provision and organization structure of a healthcare organization has to be changed in response to environmental challenge. The provision of self-pay services, such as cosmetic services, has come means of hospital to thrive during the past few years. By providing more self-pay services, some hospitals hope better off in financial statements. This study proposes a performance appraisal model based on balanced score card system to review the real case of the dermatology department of the public hospital in developing the self-pay services.
