Summary:碩士 === 國立臺灣大學 === 護理學研究所 === 90 === Abstract The purpose of this study was to investigate the knowledge of the Nurses regarding to enteral nutrition, and the interventions the nurse taken to manage the gastrointestional intolerance problems encountered. A structured questionnaire was developed by the investigator after reviewing of the literature. There were 95 items in the questionnaire and were classified into 4 areas: digestion of nutrients (14 items), NPO on gastrointestinal function (17 items), enteral intolerance (31 items), and feeding related nursing (33 items). Five experts for the clinic reviewed the questionnaire for content validity. A content validity index of 0.98 was obtained. Nurses who worked in medical and surgical wards of 4 medical centers in Taipei city were recruited for this study. A total of 719 questionnaires were distributed and 557 were returned with complete data. SPSS 10.0 was applied to process the datum, and the major statistical procedures adopted were: frequency, descriptive statistics, t-test, ANOVA and chi-square. The significant level was P<0.05. All the respondents were female, with the mean age of 27.3±4.8 years. Most of them did not have children (86.5%), received associate degree at least (62.8%), having experience of tube feeding (96.2%), but not receive any on-job education on enteral nutrition (86.2%). The most common problem encountered in providing enteral nutrition was excess gastric residual volume, which was the major reason to terminate enteral nutrition. The most common intervention to manage gastrointestinal intolerance problems was to modify the feeding method. The knowledge of nurses regarding to enteral was not fair. The overall correct rate was 47.3%. The best area was “the impact of NPO on gastrointestinal function”, “enteral intolerance” the next, and “digestion and absorption of nutrient the worst (the correct rate: 37.6%) Nurse who is under age of 25 years old, with working experience less than 1 year, and not receiving any on-job education regarding to enteral nutrition had significantly lower knowledge scores than their counter partner. Nurses who had higher knowledge scores in “impact of NPO on gastrointestinal function” and “gastrointestinal intolerance” tend to take modifying the feeding method to manage feeding problems. Results of this study indicated the necessary to provide on —job education for nurses who administered enteral nutrition for the patients. Findings regarding areas of knowledge inadequate revealed by this study provide good reference for designing on — job education. Questionnaire developed in this study can be periodically administered to nurses to evaluate their knowledge to assess the educational needs and the competence of nurses in providing enteral nutrition for patients.
