Balance deficits and locomotor dysfunction are frequently seen in patients with stroke. Some studies have investigated the effects of muscle strength training on balance in hemipleic stroke patients; however, the results have been inconclusive. The purposes of this study were to determine wheather isokinetic strength training can improve the strength of the hemiparetic knee musculature and balance in stroke patients. Thirteen chronic stroke patients(7males, 6females), aged 61 to 74 years, were assigned to either a control group or an experiment group. All of the subjects participated in a conventional therapeutic exercise program two sessions a week for eight weeks. The experimental group also received isokinetic training two sessions a week for eight weeks on their affected knee extensor/flexor along with the conventional therapeutic exercise program. Peak isokinetic knee extensor and flexor torque, the maximal sway area of posture and Berg Balance Scale scores were recoreded at baseline and after 8 weeks of training. Two-way ANOVA was used to analyze the data. The experiment group showed significant improvement in the extensor and flexor torque, Berg balance scale scores, and posture sway following 8 weeks of training. The control group also showed significant improvement in knee extensor. There were significant between-group differences on knee extensor and flexor torque; however, there were no significant between-group differences for posture sway and Berg balance scale scores. In conclusion, isokinetic training can improve the strength of the hemiparetic knee musculature, but the isokinetic strength training had a limited effect on balance in hemipleic stroke patients.
