Epidural anesthesia is usually used for lower extremity surgery, lower abdominal surgery, or postoperative pain relief. Epidural anesthesia carries the risk of hypertension and occasional bradycardia. The pathophysiology is attributed to the sympathetic blockade. The blockade can lead to vasodilatation and redistribution shock may happen. However, it’s difficult to predict the blockade related to epidural anesthesia in clinical practice. The anesthesiologists are trying to understand the change of autonomic nervous system during the perioperative period to improve the quality of anesthesia and patient’s outcome. Heart rate and blood pressure variabilities were analyzed by using off-line way. The level of blockade was between T3 and T10 from10 patients (ASA class I) by epidural anesthesia after 15 minutes. The Student’s t-test for paired data (control v.s. epidural anesthesia) was indicated the statistical significance of change increasing 10% in the value of mean of HRV and systolic blood pressure,LF power spectral density was higher 160%. The mean of systolic blood pressures of four patients was lower 12% between T3~T5 level, other parameters had no significance of change. The other six patients were T6~T10 level , the statistical significance of change included LF power increased 224%,HF(nu) decreased 15% of distribution. The results showed that the LF power spectral density can be used as marker of sympathetic activity to the heart below T5 level.
