The research employed a two-group quasi-experimental design with purposive sampling. Subjects were recruited from a medical center in Taipei. There were 45 subjects in the non-pneumonia group, and 60 subjects in the pneumonia group. The subjects in the pneumonia group were further randomized into a experimental group ( 30 subjects ) and a control group ( 30 subjects ). Subjects in the pneumonia group received a 30-minute special teaching program focusing on pneumonia prevention, Data were collected by having the subjects fill out a set of questionnaires on knowledge, self-efficacy and self-care behaviors of pneumonia prevention. The major stastistical procedures applied in data management were : McNemar’s test, independent t test, paired t test, one-way ANOVA, Pearson’s correlation and repeated measures ANOVA . We found that the pneumonia group had better knowledge about selecting proper place for activity than non-pneumonia group, but lacked in knowledge of pneumonia, outdoor activity and did not know influenza vaccine. Pneumonia subjects had the lowest scores in self-efficacy and self-care behaviors regarding pneumonia prevention, maintenance of the living environment, nutrition, exercise, physical care and symptoms of pneumonia. However, the pneumonia subjects had better scores on the knowledge, self-efficacy and self-care behaviors, regarding smoking, drinking and activity place. The knowledge, self-efficacy and self-care behaviors of non-pneumonia subjects were better than that of the pneumonia group. The scores of knowledge and self-efficacy and self-care behaviors were significantly improved in experimental subjects after receiving teaching program. This improved effect, without any difference in various background, was maintained at least for one month. Knowledge of pneumonia showed no difference in subjects with various background, but self-efficacy and self-care behaviors were related to the education level, economic resources and their daily activity. Knowledge was significantly correlated with self-efficacy and self-care behaviors. Subjects with a higher self-efficacy also tended to engage in more self-care behaviors. These findings indicated that pneumonia subjects who participated in more teaching programs had greater self-efficacy and engaged in more self-care behaviors. We concluded that a 30-minute teaching program on the knowledge of pneumonia prevention, were significantly enhanced knowledge and self-efficacy to facilitate self-care behaviors in elderly subjects. The clinical nurses and community nurses can apply the teaching program in their practice to promote the effectiveness of pneumonia prevention in the elderly.
