| Summary: | 碩士 === 國立雲林科技大學 === 工業工程與管理研究所 === 87 === The purpose of Discharge Planning (DP) is to help chronic patients leave the hospital fast and satisfactorily. When patients need to go to long-term care facilities, DP may provide a suitable transference service to help chronic patients and their sibs to choose a suitable facility. The purpose of this study is to help chronic patients and their sibs choose the long-term care facilities fast and satisfactorily. Through the bibliography and in-depth interview with several hospitals that have DPs in central Taiwan, we can find 4 factors: health care policy, long-term care facility, chronic patients and their sibs and DP, we used the 4 factors to design the questionnaires. We used the self-administered questionnaires to survey 21 nurses and 44 chronic patients and their sibs in Chia-Yi Christian Hospital. We used system archetype to find the root solutions. The findings of this research are the following : (1) Discharge planning is a very important part in the transference service, it takes charge of the communication of chronic patients and their sibs and the long-term care facilities, and provide the information to help hospital and the long-term care facilities improve their quality. (2) Nurses who make more account of the long-term care facilities are the care service of the patients, first-aid installation, sickroom installation, fire-fighting installation, and the instant of medical care. The chronic patients and their sibs who make more account of the long-term care facilities are first-aid installation, fire-fighting installation, the cleaning of sheer, quilt and clothes, kitchen installation and the instant of medical care. (3) There are significant differences between nurses and chronic patients about the medical care service, others'' opinions and the social care of patients. (4) We propose a tool to help chronic patients and their sibs to choose a suitable long-term care facility.
