This study is the first patient-oriented analysis of blood transfusion in Taiwan. The major goal was to understand the blood transfusion utilization situation and the related influential factors. It is also hoping to search for a reference index of transfusion for NHI (National Health Insurance) policy in the future. The study was based on the NHI database claimed by hospitals from July to December 1997 including 14 medical centers, 64 regional hospitals and 542 district hospitals in Taiwan.
Among total number of 1,162,282 hospitalized patients in this period, 10.9% of patients have received blood transfusion. Each transfused patient received an average of 1.46 kinds of blood component. The average transfusion cost of hospitalized patients is 584.87 new Taiwan dollars. Among all transfused patient with blood component, 14.1% patients received both red cells and plasma while 56.1% patient received only red cells. It is significantly different between blood supply shortage period and non-shortage period in blood transfusion practice. The factors significantly related to the transfusion include patient’s age, length of hospitalization, surgical procedures and kind of medical specialties (p<0.05). The transfusion reference index developed in this study for reducing unnecessary transfusion was not so feasible. Multiple regression displayed a weak relationship among the variables adopted in the analysis.
The transfusion rate of hospitalized patients in Taiwan was found higher than those of USA and Canada. This study provides initial data and findings for further regulatory purpose in the future. It is anticipating that better transfusion practice will improve the efficiency of utilizing health cares resources.
