This study is to recognize the current status of patient counseling in the ambulatory-care setting of the medical centers in Taiwan, and to investigate the necessity of this service in the ambulatory-care setting from both patients'' and pharmacists'' point of view. The study is divided into two parts: a survey of the the pharmacist and a survey of the patient. A questionnaire was distributed and collected from all the pharmacists in each medical centers in Taiwan. A one-month randomized control study was conducted among the ambulatory patients with acute disease at NTU Hospital by questionnaire and telephone follow-up. Fifty- three per cent the patients enrolled did not know the availability of patient-pharmacist counseling service, and 12% of the patients didn''t know what the pharmacist can help. Patients usually got their drug information from the doctor(57﹪), newspaper and magazines, the medical references, rather than from hospital pharmacists(22﹪) and the community pharmacists(11﹪). Ten per cent of the patients had experienced adverse drug reactions. Fifty-five per cent of the patients had been noncompliant, mostly due to missing the dose or that the symptoms had relieved. Most pharmacists in the medical centers considered patient counseling necessary, and are willing to provide the service. The barriers include heavy workload, short of time, unawareness of patient''s medical condition, and environmental interference. Improvement in pharmacy''s manpower and continuous education are vital in improving this service in the ambulatory-care settings.
