Nutrition Background: In the past decade, food insecurity rates have declined overall, but have increased significantly among older adults. Relatively little research has explored the role of older adults in relation to household food security among mixed-generation households. Objective: To explore the relationship between an older adult’s role, household food security status, and diet quality in mixed-generation households. Study Design, Settings, Participants: We conducted a cross-sectional analysis of a nationally-representative sample of U.S. households with at least one older adult (age 60+ years) from the 2011-2016 NHANES. Households were categorized as: older adult as head of household and primary child caregiver (i.e., no adults 18-59 years); head of household, not primary caregiver; and neither head of household nor caregiver (i.e., reference person < 60 years). The U.S. Household Food Security Survey and Healthy Eating Index-2015 (HEI-2015) from a single dietary recall were used to assess food security status and diet quality, respectively. Measurable Outcome/Analysis: Weighted chi-square tests were used to compare differences in food security status across household categories. Weighted general linear models were used to estimate differences among household category and HEI-2015 scores. Results: The role of an older adult in the household is significantly associated with household food security status (χ2 (3.075, 144.5) = 286.6, p< .001). Households for which an older adult is head of household and a child caregiver had significantly poorer food security status, with 23% food insecure. Older adults in this role had significantly poorer diet quality scores (mean: 50.2, 95% CI: 48.8-51.7) than those who were head of household but not a child caregiver (mean: 53.7, 96% CI: 52.8-54.5). Conclusion: Poorer food security status and diet quality of older adults with caregiver demands have implications for the design and administration of nutrition programming.
