Health Care Introduction: In the VIP study, the Lea Symbols® Visual Acuity Test at 5 feet was found to be one of the best tests to identify significant visual problems in preschoolers. The purpose of this study was to investigate the ability of the Lea Symbols® Visual Acuity Test at 5 feet to identify significant vision problems (significant refractive error [RE], strabismus, amblyopia) in school age children. Methods: Children (ages 5 to 13) were enrolled in the study during their comprehensive eye examination at The Ohio State University College of Optometry Pediatric services. The Lea Symbols® Visual Acuity Test at 5 feet was performed with no visual correction in place. Each child underwent cycloplegic autorefraction using the Retinomax Autorefractor. Significant vision problems included identification of any of the following on comprehensive eye examination: any significant RE on cycloplegic autorefraction (myopia: =-0.50D in any meridian, hyperopia: =+2.75D in any meridian, astigmatism: =1.50D, anisometropia: =1.50D difference in spherical equivalent [SEQ]), or any strabismus or amblyopia ( = 2 logMAR lines of interocular difference [IOD] if visual acuity [VA] is 20/25 or worse in the better eye, or = 3 logMAR lines of IOD if VA is 20/20 or better in the better eye). The ability of the Lea Symbols® Visual Acuity Test at 5 feet to identify school-age children with a significant visual disorder was summarized by the area under the receiver operating characteristic curve (AUC). Results: A total of 96 children (mean age 8.9 years) participated in the study. A total of 36 children met the criteria for myopia, 15 for hyperopia, 22 for astigmatism, 7 for anisometropia, 9 for strabismus, and 4 for amblyopia. The mean refractive error was +0.16 D (range: -6.85 to +6.85). The AUC for detection of children with one or more targeted visual disorders was 0.80.Conclusion: The Lea Symbols® Visual Acuity Test, when administered at 5 feet is an effective screening test in school-age children for identifying children with significant visual disorders, particularly in those under 10 years of age. With an AUC value of 0.80, these results showed very good performance for detection of children with one or more of the targeted visual disorders.
