Dental Care Class II Surgical orthodontics Decompensation Introduction: The information that details dental changes accompanying pre- and postsurgical orthodontics during orthognathic surgery treatment is disappointing and results in less than ideal surgical change, but is largely derived from university clinic samples with patients treated by residents (clinical novices). This study examined similar treatment performed by experienced practitioners and compared it to the novices’ results. Methods: A sample of seventy-two class II subjects treated by experienced practitioners (EP) with a mean of 26.7 years of experience was selected. Inclusion criteria were: consecutively treated surgical-orthodontic patients with mandibular advancement, rigid fixation and good quality lateral cephalograms. Pretreatment skeletal and dental variables were compared to a sample treated by novices. This was followed by presurgery and final analyses using an ANCOVA with pretreatment values as the covariate. An efficacy analyses for treatment phase and study comparisons was the final component. Results: The novices treated significantly more severe class II skeletal patients. For both studies, there were significant positive changes in the position of the mandible. The ANCOVA analysis showed that the EP managed the bodily position of the upper incisors more effectively. The efficacy analysis found no statistically significant differences between novices and EP. For both novices and EP, according to the ANB changes, non-ideal incisor decompensation led to less than ideal final mandibular position. Conclusions: The dental and skeletal mean changes and efficacy analysis for both novices and experienced practitioners showed that presurgical orthodontic treatment often does not fully decompensate the incisors, which then limits the surgical outcome. 2009-08-27 English text The Ohio State University / OhioLINK http://rave.ohiolink.edu/etdc/view?acc_num=osu1236704017 http://rave.ohiolink.edu/etdc/view?acc_num=osu1236704017 unrestricted This thesis or dissertation is protected by copyright: all rights reserved.
