The concept health-related quality of life (HQOL) was developed to represent the overall impact a disorder has on an individual’s life. Many instruments have been developed to measure HQOL in migraine, though the conceptualization of HQOL remains vague and no gold standard of HQOL assessment has been identified. Consequently, the construct validity of many of the most popular HQOL assessment instruments has not been adequately examined. The first purpose of this study was to examine the HQOL construct by testing the hypothesis that HQOL of people with severe migraine reflects three common factors: Affective Status, Functioning, and Pain Density. This model is based on a previous factor analysis of HQOL in people with chronic tension headache (Holroyd et al., 1999) as well as a review of theoretical constructs typically examined by HQOL assessment instruments. The second purpose was to test how well several of the most widely used HQOL assessment instruments (Medical Outcomes Study Short Form Health Survey, Headache Disability Inventory, Migraine Disability Assessment, and Migraine Specific Quality of Life Questionnaire) measure these factors and identify the instrument that best measures each factor. To examine the construct validity of each HQOL assessment instrument, externally validated measures of each factor were included in the models (Beck Depression Inventory-II, Beck Anxiety Inventory, and daily electronic diary). A confirmatory factor analysis was conducted, followed by a post-hoc exploratory factor analysis. In conclusion, HQOL for people with severe migraine appears to be composed of two domains: Affective Status and Functioning. Several popular HQOL assessment instruments have subscales which do not adequately measure their intended domains. In particular, many of these instruments do not relate to a daily electronic diary measure of Functioning. The best HQOL measure of Functioning was the Migraine Disability Assessment.
