Gerontology integrative health services integrative financing adult day services home health services The Program of All Inclusive Care for the Elderly (PACE) features integrated delivery of acute and long term care services through a coordinated effort of interdisciplinary healthcare professionals based at an adult day health center. SeniorLink, the PACE provider in Cincinnati, Ohio, has initiated a home based team of participants who receive all PACE services but do not attend the ADHC on a regular basis. This study compared participants in the home-based option to those enrolled in the traditional PACE program. Comparative and descriptive information on utilization patterns and outcomes for six months are presented.
