The Massachusetts Promise Fellowship Program serves youth throughout various communities of Massachusetts. In order to receive funding from their parent organization, it must develop performance measures and a comprehensive reporting system that collects, organizes, and reports performance data on an ongoing basis. The recommended design solution for this project is a web-based computer reporting system that was developed using the survey software tool, SelectSurveyASP. The design includes online performance surveys required by the Fellowship Program, which were all created and customized using SelectSurveyASP based on given design requirements. Fellows can access the reporting system from any computer with an internet connection to complete these surveys. The reporting system then compiles the survey results in a data summary file. The Fellowship Coordinator can access the system to view real-time results or to export the data to Excel. Finally, the Fellowship Coordinator can incorporate the needed information into the report of the Fellowship Program to request funding.http://hdl.handle.net/2047/d10011298
