Summary:<p>Background. Acute hemolysis is a tempestuous life threatening disease. Finding the right couse as soon as possible is very important for the beginning of the right therapy. Because of its quick development there is a frequent lack of time to find exact causes and to prevent its development.</p><p>Methods and materials. A case of 62-year old female patient with ulcerative colitis, sideropenic anemia and febrile neutropenia is presented. She received a transfusion of 2 units of packed red blood cells. The following day she developed massive acute intravascular hemolysis with shock and multiple organ failure. She died within 24 hours after receiving the transfusion.</p><p>Results. If the outset of hemolysis begins after blood transfusion, we have to exclude unwanted side effects of such terapy as the reason for hemolysis.<br /><br /></p>
