It is grounded on empirical investigations results ant theoretical developments undertaken since 2004. It shows that the expression witch appears in French political domain in the first years of 21st century is polysemous: change of « governance » towards relocation of empowerment nearest of population needs, civil rights of users, reshaping of patient/practitioner relation, reduce of health inequalities, destigmatization of some pathologies. The papers shows the emergence of new stages where innovative forms of local democracy, and production, share and strengthening of local and lay knowledge. But law modifications end globally in increase of control regulation, enhanced by new management software. The ambition of « health democracy » results in a splitting in the frame of actors’ confrontation.
